COVID-19 SELF-ASSESSMENT
Before calling 811 use this self-assessment tool to help determine whether you should be tested for COVID-19. You can complete this assessment for yourself or on behalf of someone else, if they are not able. Common symptoms for COVID-19 include: dry cough and/or fever, other symptoms may include: sore throat, headache, runny nose.
If you have symptoms :
- You can contact your primary care provider (doctor or nurse practitioner) or call 811
- Please do not visit a hospital, physician’s office, lab or healthcare facility without calling first.
- If you work as a health care professional, such as physician, nurse practitioner, nurse, pharmacist/technician, Ambulance NB, Extra Mural program, first responder or related dispatcher, staff in hospitals, nursing homes, and other institutional or group living settings providing direct patient care/contact please self-isolate and call 1 833 475 0724.
- If you are having difficulty breathing or experiencing other severe symptoms, call 911 immediately and advise them of your symptoms.
